This policy covers www.drlunaxu.com only. It does not cover your medical record. The Notice of Privacy Practices under HIPAA, which explains how we use and disclose protected health information in your care, is available at the office and on request.
Please do not send medical details, insurance numbers, dates of birth, or other sensitive information through these forms. The new-patient form on this site is a print-only worksheet: it is never submitted to us over the internet, and what you type stays in your browser until you print it.
We use what you send only to reply to you and to schedule your visit. We do not sell it, rent it, or use it for advertising.
Our hosting provider keeps standard server logs (IP address, browser type, pages requested, and timestamps) for security and troubleshooting, and retains them for a short period. The site loads the Inter typeface from Google Fonts, so your browser contacts Google's servers and Google receives your IP address. We do not run advertising trackers or third-party analytics on this site.
The site sets no cookies of its own, with one exception: the form handler starts a short-lived PHP session cookie used only to rate-limit repeated submissions. Pages that link to or embed Google content (for example, a Google review or map link) may set Google's cookies under Google's own policies.
We do not sell or rent personal information. We share it only with the service providers that help us run the practice and this site — our web host (Hostinger) and our email provider (Google Workspace) — and only as needed to provide those services, or when the law requires it.
Form submissions stay in the office email inbox only as long as needed to handle your request and any scheduling that follows, then are deleted in the ordinary course. Newsletter addresses are kept until you unsubscribe.
The site is served over HTTPS, and access to the office inbox is restricted to staff who need it. No method of transmission over the internet is completely secure, so please do not send information you would not want intercepted.
This site is not directed to children under 13, and we do not knowingly collect their information online. A parent or guardian should contact the office by phone.
You can ask us to access, correct, or delete information you sent through this website by emailing admin@drlunaxu.com or calling (718) 442-2711. New York residents have additional rights over their private information under the SHIELD Act, and we honor those requests. We may need to verify your identity, and we may keep records that treatment, payment, or the law requires us to keep.
Links to Google Maps, social media, WeChat, PubMed, or product pages lead to sites we do not control. Their privacy policies apply once you leave this site.
If this policy changes, we will update this page and the effective date above.
